Executive Order 13G Compliance

The information on this page is intended for district employees only.

Following the issuance of Executive Order 13G, an email was sent summarizing that all employees who are fully vaccinated provide the district with proof of their vaccination status. That communication also indicated that the district would be following up with (1) instructions for providing vaccination documentation; (2) the district’s regulations, consistent with Order 13D, the CSDE FAQ, and any further applicable guidance, regarding vaccination and testing requirements; (3) instructions for submitting exemption requests; and (4) instructions regarding weekly COVID-19 testing for unvaccinated employees.

Please find attached below the following important documents:

  • Directions for submitting vaccination documentation via Google Form.
  • Forms that employees wishing to request a medical or religious/spiritual belief exemption from the COVID-19 vaccination requirement may use to request such exemption -- attached as Appendix A and Appendix B to the above-referenced regulations.

Please note the following important deadlines:

  • By end of day September 22, 2021 (accelerated due to new State guidance), employees must provide:
    • Proof of vaccination status; or
    • Exemption request completed on Appendix B or Appendix C; or
    • Notification of refusal to receive a COVID-19 vaccination.
  • Starting on September 27, 2021, employees who are not fully vaccinated must submit to COVID-19 testing one time per week on an ongoing basis (until fully vaccinated, if full vaccination is anticipated) and provide adequate proof of the test results on a weekly basis.
